The Network consists of 21 institutes where the reference centers deal with over 900 rare diseases included in the LEA.

The Network uses a dedicated information system (Regional Rare Diseases Information System - SIMaRaL) and common care pathways that are defined in the PDTA (Diagnostic Therapeutic Assistance Pathways).

ASLs play a crucial role for people with rare diseases because:

• they facilitate access to the care pathway;

• they strengthen connections with integrated networks, proximity services, and social-health integration.

Each ASL establishes a reference figure you can turn to.

This model aims to strengthen the relationship between hospital and territory to:

• ensure patient care;

• provide appropriate and continuous assistance;

• offer a coordinated multidisciplinary approach.
